2024 APPLICATION

Dear Student,

Thank you for your interest in the Community First Bank of the Heartland Scholarship. The recipient of this scholarship will receive funding towards the continuing of their education. Below are the requirements you must meet in order to be considered for this scholarship. Applicants with a personal or family relationship with the Community First Bank will be given first consideration.

Must be a senior graduating in 2024 from a Jefferson County High School
Must have an accumulative GPA of at least 3.25 on a 4.0 scale
Must submit the School Verification of Scholarship form and your high school transcripts
Must submit three Character References with your application

1.) Please describe your involvement in school curricular and extracurricular activities. List the identified activities, citing the number of years of participation and your role and responsibility with the activity and/or organization.
2.) Please describe your involvement in community activities. List time involved and why being involved with the organization(s) is important to you and what you have gained from being involved.
3.) Describe your future college and career plans.
Develop an essay portion on the following topic: “What does your community ‘need’ and why?”
This essay should be approximately 250 words typed or computer generated.

I certify that, to the best of my knowledge, all information given is true and correct and give permission to Community First Bank to contact my high school for any additional information that may support my nomination. I understand that this scholarship is partly based on leadership and values. I agree to forfeit any scholarship money unpaid by Community First Bank should any incident occur which would lower my score for leadership/values----even if such incident happens after the press conference announcing me as the recipient is held. I also give Community First Bank the right to use my name and/or photograph in all news releases and media if selected as a scholarship recipient.
